Vitamin C in Covid-19 therapy is very important, vitamin C plays a role in maintaining the integrity of the body's immune system. The dosage of vitamin C must be appropriate to achieve effective bioavailability.
In conditions of infection and in critical patients, there is an increase in the need for vitamin C, due to ➡
- Metabolic Needs.
- Inflammatory process.
But, on the other hand there is depletion or decreased absorption of vitamin C due to the presence of inflammatory cytokines.
The role of vitamin C in Covid-19 therapy.
- Vitamin C has extensive physiological functions in the body and vitamin C is an antioxidant enzymatic co-factor that is important for the potentiation of the immune system. Specifically modulating the immune system in maintaining the integrity of the immune system.
- Vitamin C regulates hormone production in the body.
- Vitamin C requires an intermediate sodium vitamin C co-transporter (SVCT) in order to cross the cell membrane.
There are pharmacokinetic differences between oral vitamin C and intravenously.
➡ On the use of oral vitamin C : 200 mg/oral can be absorbed completely, bioavailability is 100%. However at higher doses bioavailability decreases due to saturation of the SVCT transporter protein.
Parental Vitamin C in sepsis patients :
- Can suppress the immunosuppression pathway so as to prevent organ damage caused by sepsis.
- Vitamin C parental can act as an antioxidant that increases pulmonary epithelial function and alveolar physiological ability.
Giving vitamin C, 6 grams/day intravenously divided into four doses, each of which is given :
- 1½ grams for 4 days for pneumonia patients (not Covid-19 patients) does not cause the risk of Acute Kidney Injury.
Effectiveness of high doses of Vitamin C : dose of 50 mg/kg/day given every 6 hours for 96 hours.
From various sources, high doses of vitamin C still exist that provide up to 5 mg/day for sepsis patients and ARDS patients with normal kidney function.
WHO recommends giving vitamin C to patients with Covid-19 to :
- 16 grams/day, but care must be taken when using high doses of vitamin C together with nephrotoxic drugs.
- The risk of oxalate nephropathy, especially in patients who have experienced renal insufficiency.
